This website does not keep your personal information. We do not collect your name, phone, email, address, or other personal information such as birth date. When the medical history is collected through data exchange with your medical record, we have temporary access to your personal identifiers but we do not collect or keep this information.

You can share the project advice with your clinician using a report code that is not unique to you and applies to anyone with similar medical history. You must initiate the sharing of the report by sending the report ID to your clinician. You can also print the report and share it with your clinician.

We may aggregate all responses we have received to date to examine effectiveness of this decision aid. This aggregate report cannot be used to trace the information back to you.
